Annotate Leap July disenrollment meters with workspace, contact, and live telemetry

Subtask of BOLT-1863. Grace requested a read-only production audit of the attached Leap July disenrollment CSV. Resolve each partner_reference/enrollment and Leap meter to its current workspace and contact/customer name; determine whether fresh telemetry is flowing using authoritative current device state; append columns, validate output, and deliver in the Slack thread. No production mutation.
