Combine Leap initial and new disenrollment lists with resolved fields

Subtask of BOLT-1865. Grace requested a read-only combined CSV from the attached Leap initial list and new list. Preserve the initial-list columns, query production to resolve every corresponding field for the new-list enrollments, refresh telemetry where time-sensitive, validate the combined output, and deliver it in the Slack request thread. No production mutation.
