Add .blockers.env reconciliation to nightly reflection

Ensure every nightly reflection compares each active .blockers.env entry against actual PR state (gh pr view) and Slack DM reality. Resolve stale/closed PRs and remove fabricated blockers in the same session. Update reflection prompt / checklist so the step survives compaction.
